Mobile Medication Management helps caregivers and families feel at ease, especially during the busy holiday season. This service at the CGC is available for consumers who have difficulty managing their medications and health appointments and is especially helpful for people with memory care needs. The Mobile Medication Nurses are not just healthcare providers—they are also educators, support systems, and consumer advocates. The goal of the program is to assist with medication adherence and to work toward independence. Consumers must be over the age of 18, have a mental health diagnosis, and take medication; they do not need to be consumers of the CGC. The program provides support as needed based on evaluation and coordinates care with other healthcare providers.

When one participates in our Mobile Medication Management program, you can trust that your loved one is being educated about the importance of and ways to stick to the prescribed medication schedule. This means you can run errands or take a break without worrying, knowing the nurse will assist your loved one with their medication. Our nurses provide services right in your loved one’s home. If there are questions or concerns during regular operating hours, and the nurse is unavailable, you leave a detailed message in our med box, and our nursing staff will return your call.
At CGC, our Medication Management program goes beyond just handling medications. Our licensed practical nurses have the opportunity to attend other medical appointments. This helps the nurse understand their medical needs and allows you and your loved one to build a trusting relationship with our nurse.
