Telehealth

Our telehealth services are now provided through a secure mobile app and website powered by OnCall Health. With the CGC App, you can:

 

CONNECT WITH YOUR PROVIDER
Securely log into your account (on the CGC app or website https://app.thecgc.com) and directly connect with your provider to join your telehealth video appointment.

 

SUBMIT DOCUMENTS
No need to download and print forms. Complete and sign documents electronically with the HIPAA-compliant app.

 

VIEW UPCOMING APPOINTMENTS AND RECEIVE APPOINTMENT REMINDERS
Never miss an appointment! View upcoming appointments and receive reminders (text*, email, or phone) when you create an account. Of note: to receive appointment reminders, you must have an account.
*Text or SMS standard rates may apply.

 

By accessing your provider using the CGC App, OnCall website or your provider’s Zoom link, you are consenting to Telehealth Services provided by the Community Guidance Center (CGC). If you are not consenting to Telehealth services, please do not log into your account via the CGC App/website or click on your provider’s link. Call CGC at 866-686-1991.

**If your email address is on file with CGC, you will receive an email confirmation at the time of appointment scheduling, and a reminder 24 hours in advance of your appointment, and again one hour in advance of your appointment.

 

You must wait for this email to create your CGC OnCall account.
